Initial Comments:
Based on the findings of an off-site unannounced follow-up survey completed 8/15/2023, Omni Home Care had corrected the deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a Medicare recertification survey completed 4/27/2023.

The deficiency was cited as a result of a State relicense survey completed 4/27/2023.
